In 2000, when I was 20 years old I blew a .04 and was cited with a "minor operates a motor vehicle with a BAC above .02"

In 2006 I had a DUI and blew a .22

In 2009 I had a class 2 flight physical, and REPORTED BOTH INCIDENTS on the form and told the examining AME. He gave me my Class 2.

I just got mail from the FAA in Oklahoma, asking for additional paperwork (police reports, treatment records, and my driving record).

I heard that "evidence of substance dependance/abuse" is a disqualifying condition. Also that anything above a .15 BAC makes it even worse.

Will I lose my Class 2 Medical?

If it is an isolated incident, probably not. If you have another one, you might lose it. It depends on the FAA reveiw.
