Which is more the reason why you should always file an ATSAP for OE/ODs that were not caught by anyone other than yourself. Now if you committed an OE/OD and you didn't know you did, as soon as you are made aware of it, you need to file the ATSAP.

This is the comment I was going to make. As soon as you become aware of a situation (whether as it happens or after the tape sessions) you have 24 hours to at least start the process for ATSAP, assuming your facility is covered under ATSAP of course.
