My friend just informed me that he got one in the mail today about me. So I guess they're sending them out and everyone we put down can start expecting them.

No word yet on my end. I wonder if the forms were still sent out even though there is something pending with medical....hmmm we'll see, I guess.

One of my friends got one too. She is one of the people I listed as "3 other contacts." I am from Maryland btw.

I wonder when our employers will start getting theirs. The form is very basic, just like everyone else said, basically a scan-tron. Yay! Another step in the process!
