Not sure if this is the correct forum for this particular question, but I am curious as to how long the FAA has mandated that you submit this with your application?

Also just curious if anyone has any examples of how they filled theirs out. I'm more so curious how factor 1 is filled out. And if its better to fax or mail? And if fax or mail how exactly did they go about doing so? TIA

I also have several questions about this form:
1. Do we put our own scores in the top right corner?
2. What is the 'Selective Placement Factor'? Options are 'Satisfied', 'Not Satisfied', or 'Not Applicable'. I'd be tempted to check 'N/A' since I don't know what it means, but it says in the instructions "Candidates who have not satisfied appropriate selective placement factors are not qualified for the vacancy." I found this link from a Google search, but it didn't fully clarify the subject for me, and if it applies to the bid I am applying for.
3.FACTORS 1: Another employee showed me their example. Under the 1st box labeled 'Position, Title, ATC and Career Level' they listed on separate lines for one facility ATCS FG, MMAC, ATCS AG, ATCS DEV-2, ATCS DEV-3, and ATCS CPC. I'm not sure if this is right.
Under 'Facility', 'Fac. I.D.' is your 3 letter identifier. For 'Type' I am unsure what to put for an up/down facility. The examples listed on the instruction form for 3330-43 are "ARTCC; FSS; CS/T; CAB; Radarcab (R/C); Radar Approach Control (RAC)." The consensus at my facility is 'RAC', but does that include Tower, or imply a TRACON only?
What is CS/T and Radarcab (a facility that is always TRAcabbed)?
4. FACTOR 4:
b. - 'FAA Correspondence Courses'. Anyone have examples? Would this be courses like RTF in OKC, or the class to become an OJTI? Anything on eLMS?
c. 'Long-Term Training'. Examples?

3. List every time you have you received a pay increase. For an up/down list tower/tracon.
4. I listed RTF and my manager agreed but I'm not sure about anything else.

I thought we needed to put one of the listed abbreviations. (ARTCC, FSS, CT/S, CAB, R/C, or RAC). 'Tower/TRACON' seems too long to fit in the spot. Again on the 3330-43 instructions it says: "c. Facility type. ARTCC; FSS; CS/T; CAB; Radarcab (R/C); Radar Approach Control (RAC). Enter the acronym for the facility type on the line with the position title."

That's what I put down and I got picked up so it worked for me.

I have been working on filling this form out as well... seems more difficult than it should be, perhaps it is the poor directions (in my opinion). I was told at my facility that for 4b you can include eLms courses...

I agree it definitely could be more clear cut. I've looked at several coworkers forms for reference and each person did things just a little bit different but there never seemed to be an issue. I've never heard of listing eLMS courses though, that seems a bit ridiculous.

A couple questions about this form
1) who is the rating official? Is this person at your current facility, the other facility, regional office?
2) for 4b, what are the common courses that controllers have? how many points is those courses? it says maximum 2, is that 2 courses or 2 points?
3) for 4c, someone listed RTF, how many points does this count? The maximum points for factor 4 is 25 points and the maximum for 4a is 20 points.

3. I didn't list Tower class & RTF, but I know ppl that did. If you have checked out in a tracon, it's a given that you went to RTF. RTF gives you no points.

I didn't go to a stand alone TRACON. I'm at a combined tower/TRACON facility. When I went to RTF, I was permanent and no longer a temporary employee so would it count for any points?

I put 5 points for RTF and got picked up. My FLM wasn't sure about it and couldn't find an official answer but signed off anyway saying it'll get kicked back if it's not right. No one questioned it. From my experience the instructions to the form and the points in particular are very confusing and no one seems to know exactly what counts for what. As long as your FLM signs it ands there's nothing obviously wrong it will probably be fine.

I also put RAC for type even though I'm at an up/down. Who knows. Like I said, no one questioned anything for me.

How does this form work if I am in the Military? I have no idea what level my facility is.

Anyone have any new insight on the correspondence courses? Is this something that hasn't been offered in years, as I have been told?

Question, I am trying to fill this out, and it states don’t list Military experience, just Civilian Experience. I Worked one year for the FAA contract tower, and worked 11 years for the DOD on the civilian side, and recently transferred to the FAA 2 years ago. It asks for the level of facility you where rated at, now for the contract tower and DOD, we did not use the level of facility rating system like the FAA does. Any ideas of what I would put in that section?

If it was me, and I wanted to include that in the experience section, I'd just list it as DOD or FCT.
